Abandoned remediation is rarely laziness. It is what happens when ownership is a group, a role title, or “the team that owns the system.” An audit issue tracking app can only escalate to a person. If that person is vague, the ticket ages in silence.

One accountable name

Allow contributors and reviewers, but keep a single accountable owner field. When two people share accountability, both assume the other will update status.

Hand-offs as first-class events

When someone leaves or changes role, require a hand-off comment with remaining work and links to draft evidence. Treat missing hand-offs as a control failure, not an HR footnote.

Capacity before commitment

During intake, ask owners to confirm a target date they can defend. Inflated optimism creates false aging metrics. Better an honest sixty-day plan than a decorative two-week promise.

Close the loop in the same record

Verification should live on the finding, performed by someone other than the owner. If verification happens only in email, your tracker will always look more open than reality — or worse, more closed.
